Narrative Example |
Chief Complaint |
The patient presents with acute mid-back pain radiating to the groin. |
History |
The patient is a 54-year old male presenting with acute pain in the mid-back region. The pain began a couple of days ago, emerging suddenly without any apparent inciting event and was initially described as a dull ache noticed while going to sleep. Over time, the pain has progressively worsened, and the patient describes it as a sharp pain on the right side, radiating toward the groin. Tha pain is reported to occur in waves and is unprecedented by any experience the patient has had before. On a pain severity scale from 0 to 10, the patient rates the pain at a 7-8, indicating significant discomfort. The patient attempted symptom relief with ice application and Tylenol, neither of which provided effective relief. The only relief experienced is while resting in bed, which is challenging due to occupational and familial responsibilities. Notably, there has been an increase in urination frequency, with observations of pink-colored urine. The patient denies any weakness, numbness in legs, or changes in bowel or bladder control, and there are no reports of fever, chills, or body aches. |
